Emergency information wallet cards for children

The Family Emergency Plan folding wallet card for children from www.ready.gov is very comprehensive.  Please be aware that each adult would need to carry folding cards for every child in the family in order to have medical information at hand.

There is no designated spot to add contact information and phone numbers to locate a child at any place other than his/her school. The “Important Numbers or Information” section could be used for that purpose, however it is usually utilized for names and phone numbers for the child or a supervising adult to contact in case of emergency. If using this section for names/numbers where you might locate your child, that distinction should be clearly made on the form.
